By signing this waiver, I recognize that I am the parent or legal guardian of the child that I am allowing to participate in the activities of the Trinity Youth Group Lake Day on July 31st at the Munao’s home.
I understand that certain risks are inherent due to the nature of it involving water and water activities. These risks include, but are not limited to, injury, disease, or any other physical threat or bodily harm. I understand that there may be a great variety of other risks not known or reasonably foreseeable. I acknowledge that Trinity Church, and the Munao Home, 1840 Gertrude place LLC is not responsible for any harm that may occur.
I fully release and discharge the Munao Home, 1840 Gertrude place LLC, and Trinity Church from all liability in connection with my child’s participation for this event.
