Because we value the safety and well-being of every AWANA leader, we ask that you share an emergency contact. Should an unexpected need arise while you are serving, this ensures we can quickly reach someone you trust to support and care for you.

We are grateful for the ways God equips His people to serve the next generation. If you have previously served in AWANA or other children’s ministries, please share your experience. Include your role, the church or organization, and the dates of service. Your history of service helps us celebrate how the Lord has used you in the past and see how He may continue to use you in this season.

I, the undersigned, am committed to serving the TEFC AWANA Club with my time, talents, and spiritual gifts. I agree to be a Christ-like example in both speech and actions to all clubbers, encouraging their spiritual growth and modeling integrity in my conduct.

I understand that as a condition of serving, I must consent to a background check conducted by Trinity Evangelical Free Church through Protect My Ministry Mobilizer. I acknowledge that my Social Security number and other necessary information will be used solely for the purpose of completing this background check. All sensitive information will be securely protected, with Social Security numbers blacked out once the check is complete, and all forms destroyed after processing. I understand that all volunteers must pass a background check to serve with children at TEFC.

I also acknowledge that participation in church activities may involve exposure to COVID-19 or other communicable illnesses. I understand that exposure is an inherent risk in any public or group setting, and I voluntarily assume this risk for myself and my family members. I release and hold harmless Trinity Evangelical Free Church, its staff, volunteers, and leadership from any liability related to exposure or illness. While the church will take reasonable precautions to maintain a safe environment, I understand that no measures can guarantee complete protection from infection.
